But i will reiterate i do not think had he been this ill and suicidal his family and his bff would have left him alone. It's impossible. If there were signs and if the docs would have told them also , I refuse to believe they would have left him alone.
I'd like to share an anecdote. In my residency we discussed a real life case that our psychiatrist brought up as a teaching point. The question was, should we as general doctors refill antidepressants without following up regularly with pediatric patient (15 years old) if the family is asking for refills, the patient has been seen by a psychiatrist and family feels that she's doing ok.
The case was just that. A general doctor refilled an antidepressant because the 15 yo girl was started on it by the psychiatrist and just needed refills. He talked to the mother and the mother reassured the doctor that the girl as doing very well and was responding to the medication. A few days later, the girl committed suicide.
Mother sued the doctor with claims that the antidepressant prescribed increases the risk of suicide and the case went to court. Doctor's defense was that the suicide could also have been caused by worsening depression, not just the medication. But in court, the doctor still came under fire because he should have talked to the patient directly and assessed for suicidal ideation before refilling the medication. The physician should have seen the patient regularly, frequently to make sure she was responding and not just relied on family.
Moral of the lesson for us: DON'T JUST TRUST FAMILY, TALK TO THE PATIENT. If mother, father brother and all are saying patient is doing well, we still have to talk to the patient. Because sometimes they don't know. And for this this was so terrifying. Because I had refiled depression medications multiple times for young patients based on family requesting for refills and knowing that the pediatric psychiatrist had started the treatment. Teaching point for me.
