In my opinion, everything points towards Aditya causing the accident. My reasons for thinking so are :

1. Shagun's suicide attempt, which was to blackmail Raman, but Aditiya doesn't know that, as per him, because of Ishita, his mom was insulted by Raman. So maybe he wanted to show Ishita how it feels to have your mom literally on death bed.

2. Raman's talk about Aditya's birth and his telling Ishita how mad he is and always have been when it comes to Aditya, who is his first born, his first son.

3. Another indication is Romi's behavior the other night when Ishita confronted him about causing Amma's accident. He looked scared more then guilty. And I feel he would only cover up or lie about the accident if he knew if one of his family members, especially Aditya was involved in it. He might have been passing by when he saw Aditya causing the accident and he must have taken Adi's car and told his friends to drop Aditya off home safely.

Adi I guess, so that now when Raman and Ishita are coming closer. May be adi's custody or Adi moving in with them will create problems in their life.

Quite a few people have been saying this, but I think it would be quite a bold step for them to show a child doing something so horrendous...even for a 11 PM show. Only for that reason, I dont think it is him. I don't know if this is a norm though, I havent watched a lot of shows.

about point # 3, there is a different explanation for it... its something Aly said in an interview... its kinda spoiler, so i won't say it here

If it's Shagun or Adi it would mean that Adi will soon become a part of the Bhalla household which is too soon. So I would like to believe that it is Param. This revelation might also lead to dumb Simi's eureka moment and she will realize her husband is not a devta. She will kick his ass and this will explain his temporary exit from the serial as he is now shooting for Kumkum Bhagya. I hope it's Pervy.
