I am going to go all out and give something else that cooked up.
Khushi is the killer. She has obsessed with the whole psychology of crime and in the process of her fervent need to write crime that is spotless she has developed this bipolar nature. It would mean a lot and make sense of her words to Arnav which I was reading again because she never invests in the murders itself, which would point to her having fictionalized her deeds enough and invests a lot in the psychology, making it clear that she enjoys the thrill of the crime itself and in an odd way justifies it. Something both the doctor and Arnav know and all of this is just a ruse to get her to normal for she would be outrageously dangerous if approached in any other manner. But this theory would make the half right bit of my previous one also to dissolve. So I guess I will have to think deeper.
