It was a little obvious, actually i tried to make it obvious that Zoya would blame Asad because after all he was her Savior, so he was supposed to svae the baby but he could not

Actually since Zoya was physically abused by Haseena, she was weak and had itenal abdominal injuries, they were just healing when Akram had slept with/raped her but when he did that heinous act with her, her injuries got even more fragile, they had started to heal when she had come to the mental asylum but then the fact that she was pregant and the baby had started to grow in her womb had made her een more fragile and then as th baby grew, her health deteriorated but then here she couldbt take it anymore and they had to 'remove' the baby (i'm not a doc ! 😆 )
