I'm guessing he came later, as per shooting request, as the spoilers are saying Radhaji and Madhu are heading for the orphanage, Sultan overheard where she is going and follows, so he arrives first they did the cutting of the cake and then RK turned up later at the same place. Not sure which is part of shooting possibly the part were Vivian is in the picture, as per the story in the show Madhu wants to go there as part of her gifts of her wedding, charitable cause, RK probably knows about and follows later, I hope he doesn't see Sultan is all I'm hoping for.
