It was a suicide. Payal had come to terms with her one sided feelings towards Arnav and then gone on to fall in love with Akash. Senior Mr. Raizada had been under the impression that it was Arnav who had been in a relationship which he hadn't corrected and used that to exert his will. God, what a dysfunctional family. I think Shyam marrying Anjali was to ensure that the ties between the families don't break off, when Sia's truth comes out. I don't feel any empathy towards Akash, probably because it doesn't look like he really put his foot down when needed, but we can't discount his sufferings too. But Arnav is the one who has suffered, a lot, for no fault of his and he has had to carry the burden of guilt on too many fronts. Payal had walked in to that relationship with her eyes open, but I think the accident probably caused her to take stock of what she had gotten into. One feels bad for Sia too, she didn't choose to be schizophrenic, it's not in anyone's hands. Here the kids became pawns in the hands of parents who should've known better.
Its too late because she's totally in love with him and nothing is going to change that fact.
P.S: It goes without saying that Shyam was behind the phone call. I remember voicing out for one of the early chapters, whether there is a connection between Shyam and Rishabh because of the political background. It seems plausible.
