I heard the dialogue bent Sahir and Kurtiapa twice in today's episode and he definitely says Mera dard meri Api hai. So I think Zeenat is the sister and not wife.

The name is Zeenat Saahir Chaudary. For siblings they can have the same surname Chaudary but nobody would keep the name of the brother Saahir as middle name or surname.

He said "mera dard mera apna hai" and if she was his sister, I don't think she would have Sahir in her name

Zeenat is Sahir's wife and the doctor clearly told. Just because you do not want her to be his wife the fact can't change. Zeenat is Sahir's wife and no doubt in that. Doctor clearly said as Zeenat Sahir Choudhry. Pls watch the episode again.
