If they wanted to remove Anandi and Shiv from Nandini's life completely then why didn't they kill Anandi along with Shiv?
They could have killed them off together after the twins were born.
They have shown in great detail her relationship with the family she landed when she was a baby and showed her pining to have her family while Mangala trying her best to impress on the child that she didn't need any one other than her (Mangala).Just because the child chose her mother whom she always wanted irrespective of what she felt for the wanna be mother she is labelled as ungrateful.
The child's mother died while trying to save herself and her children from man who is after their lives the so called family is not around to take care of the childrent for reasons not known. Just because the child considers her dead mother'smemories precious she is considered to be ungrateful.Why is she expected to be grateful to the woman who tried to keep her tied to the miserable life with that family from hell.Why is she expected to remember a woman who never cared about her wishes and desire to re unite with her family?
She had to choose between a woman who is her mother and wanted to give every thing in life but lost her unintentionally when she was trying to a good deed and a woman who expected her to cling to her a woman who tried to keep her away from her mother and the life which belonged to her. She chose the former and for that she is labelled as ungrateful?

Nandini was playing teacher student game too quite often.
Nandini became doctor to fulfill her mother's wish and also because she realised how she can bring relief to people in pain when she removed that bullet from her mother's body that night.
I think they couldn't get through Jagya .Looks like Anandi's treatment was delayed or she didn't get proper treatment because they didn't have money and may be that was the reason why Anandi told her to work towards providing free medical services for people in need who didn't have money.
DoctorSA and his wifesa don't seem to be the reason behind her choice of profession.Anandi was the reason behind her wanting to be a doctor.

I don't think doctors get slapped but they do get physically assaulted quite often.Especially in emergency medical services because that's where everyone is stressed.Doctors because they are short staffed and inadequate infrastructure needed to cater to patients, Patients 'relatives are stressed because they are worried about their relatives. The relatives resort to violence out of tension.Unfortunately these things are too common.
