First one.. not all love stories are Sunshine's and flowers. There are some which was worse than anything. This one such kind.
Second... She made it clear that she is mentally ill, that means she can't stay in a certain mood and mentality, it'll change accordingly to the situation she is in.
Third... He is the one who wants her, she is ready to sacrifice her love for him for his happiness. She knows he can't be Happy with her. That's not her insecurities, it was feel which she have.
