I don't doubt the fact that he loves Karan. That is fairly clear. But this would have been going on for years. Even Karan's defenses would have been buit up over time. It is still slightly illogical as to why nobody in the family tried to find out more. Kunal would have been young himself earlier so not him then but others? And later when Kunal grew up? I don't think he tried enough. Also, that's the point na, doesn't Kunal know the disease is infectious? So Karan refusing to hug shouldn't be surprising for him. Another thing, Karan and Sandhya are not the only source of people to find out about his disease. There is supposed to be a doctor involved, there are medical reports etc. So no matter how much Sandhya manipulates, they can find out something. Infact, I think Sandhya is lying about some stuff about his disease too maybe, so I don't expect the family to know the truth but they don't even seem to know Sandhya's version well. I think despite their best intentions, there was apathy towards Karan. And the blame is on everyone, more on the elders but Kunal and Khyati as well since they have been grown ups for quite some time now.
I don't think Kunal knows the disease is infectious... why would Kunal try to hug Karan if he did? Plus Dadu forcefully hugged Karan in the same scene. So its hard to know what Dadu and Kunal know.
To me that first scene between Karan and Kunal, suggests that it was a vicious cycle. Kunal reaching out and Karan balking causing Kunal to retreat leading to Karan distancing himself even more. All orchestrated by Sandy. I don't think it's fair to blame Kunal or Kyathi any more than to blame Karan himself. Even Karan could've taken some initiative on his illness the way Naina did.. but he was made to feel it was useless. In the same way I feel probably Kunal was made to feel the same. Also from Kunal's conversation with Meghna... it seems that Kunal felt that Karan resented him for being better at activities.. in that situation he may have felt it was better to keep a bit of distance so that he doesn't exacerbate Karan's issues.
If I had to question anyone, the person I would question is Dadu. Sandy had a hand in raising Kunal, Karan and Kyathi so I think its reasonable for them to trust Sandy implicitly. If she is handling the case its not likely that they would question whether everything that can be done for Karan is being done. But Dadu doesn't seem to trust Sandy implicitly. He seems to recognize that she has a negative side... that's why he warned her about not being negative about Sharda. That's why I feel there might be more to the story with Sandy. Like something Dadu feels guilty about preventing him from suspecting Sandy the way he should.
