It could be sawasi guathum or whatever her name is. b/c, remember her husband said that she also had a memory problem or something, so maybe she made up a fantasy world which is based on kkusum's life. (it's kinda far-fetched, bt in the world of etka kapoor, anything is possible)

Or maybe, another theory, maybe this kkusum isn't the real kkusum. Like she may be the real sawasi guathum, and the real kkusum is asking her to do all this so she can catch mahi. Like maybe swasi really had plastic surgery, but it wasn't a big surgery. And she couldn't remember anything b/c she really has some kind of problem. So maybe the real kkusum was in the same hospital or something, and she met up w/ swasi and they came up w/ this plan to catch mahi, or at that time, try to find out who was trying to kill her since kkusum still thought mahi was dead. And the real kkusum has swasi "kill" abhay. (HE falls in the lake, so he'll probably survive. Afterall, the dead come to life in etka's dramas) Anyways she had swasi "kill" abhay so she can be reunited w/ him and they can scare the hell out of mahi!😆 Now that i've written it, it sounds wierd😆

I think you are right, because even I read something that said that this is a duplicate kkusum and they are going to get mansi joshi back. But I don't know what is right😕. In a interview, mansi said that she is coming back for sure but doesn't know when. That might be true.
