Abhi even without Gap should be 19 + 4.5 (mbbs) + 1 - 1.5 (internship) + 3 (masters of surgery) + 3 (cardio thoracic specialization) + probably 1 year atleast of practicing surgeon to showcase his skill to be famous {minimum } = 31 approx.

At the start of gen 4 , 22 years leap was taken . So abhira must be 21 at the start of gen . Abhir n charu both interned together so must be of same age. Rohit ruhi must be of same age as it was an arranged marriage and even today , No family approach a girl older to their boy for arranged marriage. Ruhi abhir both were 7 -8 year older than abhira , so they were already 28-29 at the start of gen .

Just being realistic , ppl do fall in love with huge age gap , but marriage is very different n need much more than love to work out.

i think in the initial episodes after rohit-ruhi marriage,there was a scene where manisha asks ruhi to not address armaan by his name and vidya said that they are of the same age something like that,I don't remember the exact scene tho.
