If we look back, daima told that oberoi Saab has taken his secretary help to get the waaris. And also it was shown that kamini and Shakti were classmates. So what I doubt is the lady who was held captive was the secretary of Shakti.

Maybe Shakti and kamini dated when they were in college.but at the end he might have chosen to marry pinky according to his parents wish to be in good books in the family. Then there might be a delay in pinky conceiving a child. Maybe during that time Shakti might have gotten into a relationship with his secretary for getting a waaris. And here kamini on the verge of getting revenge for the abandonment might be keeping a track on Shakti and pinky life. Kamini might have come to know abt secretary bearing shaktis child and then kidnapped her. Then kamini herself might have gone to shakti and said she was pregnant with his child. At the same time pinky must have also conceived.

After delivery pinky might have delivered twins which is unknown to the family and secretary might have delivered a still born kid who is picked up by kamini as her son.then she might had swapped the kids to look like pinky gave birth to still born child. Of course doctors and nurses might have played along with kamini for money. N now Shakti in order to get a waaris at the earliest had decided to take kamini help by taking one of the twins at some cost.
