Swarga is the place, where a soul enjoys fruits of his punyas. Once these good deed's account is over, that soul is reborn.
Brhma-loka, Vailuntha, Shiv-loka are different planes. A liberated soul at the end of the life merges with the Supreme Consciousness, not to be born again.
This Supreme Consciousness is Brhman, believers may know them by the names Param-Shiv, Narayan, Para-Shakti etc.

Punishment doesn't neccessarily come in the form of hell.

Karma and the fruit of your karma is how people are punished. Yuddhistir paid heavily for staking Draupadi. He lost his son, etc.

As I read somewhere.Reaching Brahmlok,Vaikunth doesn't yield you liberation. You'll get a chance to live in that loka in the presence of god.Liberation is beyond all this...where you get merged with the god! You're not different from god,you realize that you and god are one and everything is filled by all prevaiding god.

Liberation is different from reaching Vaikuntha...Will try to post details asap.

Here it is...

Five kinds -

(1) Saayujya or Liberation (merging with God...all prevaiding, nirakar, nirgun god, this is the true form of god which is conscience and bliss), the Bhakt becomes one with the Lord like sweetness is to sugar, light is to sun.

(2) Saaroopya (Conforming into a particular form/God's image), the Bhakt gets the form of interest like that of the Lord.

(3) Saameepya (Coming close to God without merging into Him), the Bhakt remains in the close proximity with the Lord.

(4) Saalokya (Living with God in His Lok), the Bhakt remains in the Lok where Lord Vishnu or Shiv resides like an inhabitant of a state. and

(5) Jeevan Mukta (Liberated while alive). They become one with god when they are with body.Live without any bodily consciousness.Hence called as Videhas.

(Eg : King Janaka)

Yes reaching to all these Lokas is not liberation.
Merging with the Supreme Consciousness is liberation. One who is fortunate to realise this, becomes Jeeva to Shiva, is Jeevan-mukta. Just by mere thought such a person goes in Samadhi. Sees the entire Universe as a play of different Shaktis. He becomes one with Param-Shiva. This state is where Shiva-Shakti are one.
This Supreme Consciousness has different names given by their believers like Param-Shiva, Narayan, Para-Shakti etc.
Some souls realise this at the end of their life.
Those who reach at higher Spiritual level get to experience these states for some moments. Which gets them more inclined towards merging with their final goal.

The Pandavas went to the highest heaven. The Kauravas and their allies also went to hell to atone for their sins, but then they all went to Indra Lok, which is one of the many heavens. After suffering hell for a little time, the Pandavas surpassed Indra Lok and went directly to Vaikunta, the highest form of heaven, where they merged with Lord Vishnu.
Both the Pandavas & Kauravas suffered hell in accordance with whatever sins they committed, but in the end the Kauravas, their supporters, and even the Pandavas' supporters went to Indra Lok whereas the five Pandavas & Draupadi went to Vaikunta.

So i read the last parva again and it has been clearly stated, two times by the gods to yudhistira that seeing karna, arjuna, bheem, nakul, sahdev, deaupadi, uppandavas etc. In hell was just an illusion and that they have all attained their rightful places in different regions with gods.
