It's not that Raman considers Shagun and Ishu to be same...It's just from his perspective breaking fast is not a bad thing be it Shagun or Ishu and he was probably worried abt Ishu
The only reason he would have the flashback he would probably be reminded that not only is Ishu. different but she is t thousand times better

its VERY NORMAL for him to compare this vrat situations...next time KC ke waqt yeh doubt nahi aayega..!! plus ishu is not well in show too-so thts added thing which would hv tend him to ask her vrat before time
