First, I don't see how anyone can cause that kind of wound with a nail. Nails are pliable, how can they pierce the skin and disappear within? But if we're asked to believe that the nail extension broke and cut psych, then I suppose it happened when she tore her sleeve. Then it festered and turned septic, at which point she got sick from it. The fact that she's been hiding it means that wound is connected to the molestation shenanigans.
