Your details about Bharat is partly correct - after conquering Gandhara, he stayed with his sons for 5 years and helped them found the cities of Takshashila (Takshila) and Pushkaravati (today's Jalalabad). However, his sons weren't born during that time - they fought alongside him in the conquest of Gandhara, alongside Kekaya-naresh Budhajit (son of Yudhajit) as well. Note that this was after the return of Kush & Luv, & passing of Kaushalya, so assuming that Mandavi had her children the same time Sita did, that would put their ages well above 12 (since all this happened well after Kush & Luv returned, and they were 12 when they did return).
As for Lakshman, Karapathudesh was described as being in the foothills of the Himalayas - I've tried to find out whether it's in modern Nepal, but so far have seen no data to that effect. Lucknow is nowhere near the foothills of the Himalayas. Also, the name Lakshmanpur doesn't sound like one that they would have come up with then - usually, the names ended with vati, like Kushavati, Pushkaravati, or other variations. Lakshman's sons - Chandraketu founded a city called Chandrakant, while Angad founded a city called Angadvad. That was Lakshman's last activity in life - he stayed with his sons for 1 year and helped them consolidate their control, and then he returned to Ayodhya.
If Urmila was alive at the time Lakshman had his final encounter Durvasa, she'd almost certainly have become a widow, since Lakshman did not go to Vaikunta alive. I agree with you that I don't think any of them ever became widows. Someone on Youtube theorized that just as all 4 brothers were a part of Vishnu's avatar, all 4 sisters were part of Lakshmi's avatar, and they passed on with Sita. Of course, that contradicts the known account of Sita's passing, where had she taken Urmila and others with her, that couldn't have gone unmentioned in Valmiki. More likely, they passed on sometime after that.
