Yea he got an internal injury during the fight with vikram but he didn't know about it till it started hurting. Despite the fact that it starting hurting he still didn't get it treated neither did he go to the doctor. That's probably why the injury is acting up now. Its normal for an injury to act up if not treated on time.

It hurt the very 1 st day when he got back as i remembered Shireen had touched him there and he had squirmed with pain, So load of crap that he didnt know about it.
