Forum Member Ranking

I am sure these questions have been asked many times before but I can't remember the answers (ooops) but the questions are:

How does the member ranking work?
Does it depend on number of posts or quality of posts?
Who determines the rank of a member?
It is automatic or does the forum administrators/moderators determine the ranking?

Just curious.
