They showed shiv can not operate on his dadi coz she is his family member..

Also, gauri delivered a baby before she died... It doesn't seem it was shiv's.. Otherwise they would have at least hinted that in dialogues if not shown him/her..

May be Gauri loved someone else and Shiv helped them get together. And Kashyaps were angry at Gauri because of that. We still don't know what role Shiv's mom played in all this.
