Wasn't it an emergency case ? and moreover we don't see their day to day work neither the scene was shown in detail Perhaps Lal too reached there after other doctor's call they share their evening duties otherwise we have seen lal at home too at night especially when JaGan were living in his house Lal was in his home at night so its quite clear all the doctors have their duties at night
