I felt the same. And i feel it's much needed reality check for akshu. Currently abhi is stopping himself considering abhir's health and akshu is emotionally manipulating him. It could be intentionally or unintentionally only writer can tell. But her asking abhi to choose between abhir and roohi looked odd to me. Abhi is already in guilt trip. He is not even sure how to keep everyone happy.
