And Monami's concern is Karan doesn't want to be with her, he doesn't want to fight his insecurities or issues or discuss with her & figure out a way to be with her. I think its rather that she thinks that he doesnt see himself as worthy of her love(or anyone else) because he is too consumed by guilt of his brother's death. He wants to continue staying alone in that shell. He only said that he lost his heartbeat once and it will never beat again. He would rather be alone than go through the pain again.

She told Sid too that Karan's priorities are different,where her love has no place. This hurts her because she knows he loves her but doesnt want to allow himself to love her toh usne samjhauta kar liya but still she couldnt help herself from telling him that she always wanted him to hug her like this,she was wishing for a different time where she would have enjoyed his hug,not realising that Karan was hugging her as his everything.Something very previous to be cherished.
