Apart from the one point you've mentioned (about how it's impossible to miss signs of emphysema) everything else is spot on!

the language... some of the clauses you've used (your text doesn't allow me to quote or I'd have given you a longer review😳)

He'd not believe a single word unless it was from his superhero

how she rocked herself into a crying stupor and the kid had to muffle the sounds with a pillow

where she kissed him goodnight for the last time

how she didn't know if she'd passed the 'artificial smile' test.
