I think you misunderstand about ruku asking for the second child, what hely is saying after the twins die and jodha will get pregnant again with salim ruku will not be able to ask for that child, because she has already used up her wish.

I think she over played her hand, she is so gereedy for power she lost sight of what is important, she is already looking at the finish line when the game have not even started. They babies are not even born and she is plotting to gain power, what will happen when they die.
