Because zubair tells aaliya to send usmans to the office..to see zain new office after decorating it... If aaliya had not sent him then he wouldn't be in the state that he is in..hope that helps

Usmans reaches the office and goes in the elevator and the elevator starts to fall and break down and usmans collapsed..then next u see him being taken to hospital with blood all over him and suraiyya blames aaliya for sending usmans to office as accident would not have happened if he didn't go office and surayya is more upset because usmans wasn't talking to her at the time and was angry with her for accusing aaliya of having and affair with zubair and not apologizing to aaliya..

^^there was an accident with the lift - which resulted in Usman being admitted in hospital in a critical condition.
