In USA for instance, NBC, CBS and ABC are free channels. I can access them with just an antenna. But if I buy a cable box from my provider, the basic cable package will include these channels plus other basic cable channels. Same in Pakistan. ARY, Geo, Hum are Pakistan's CBS, NBC and ABC. You can get these channels with an antenna If you have a cable box, your package includes ARY, Geo, Hum.

Note that formal cable operators tend to be in big cities. But even in big cities, there are several illegal cable operators.
I don't know what Hum revenue sources are. Maybe it's subscription revenue from international markets where Hum may be accessed via cable subscription only. I see Hum is available in multiple international markets (https://www.humnetwork.tv/international/). But at least for a viewer living in Pakistan watching linear TV, there is no difference in access between three.
