In flashback Pratha mother told that the one who become Shesh Naagin is Shesh Naag destined wife.
They need to perform a ritual to become Shesh Naagin and Shesh Naag. Mahek was supposed to become Shesh Naagin, but Pratha mother forced Pratha to drink the milk so she became Shesh Naagin instead of Mahek. Shesh Naag and Shesh Naagin are just titles, not blood related. They will eventually get old, dies and new naag, naagin will replace them after performed the ritual and drink the milk. Remember Ekta doesn't believe in incest when it comes to naagin. They are allowed to do whatever heck they want to do.
