My theory:

Ishana will tell Om her boyfriend is abusive and her parents have a sour relationship so she has no one to lean on This way Om will try to become her pillar and would want to support her while she'll continuously scheme to make him fall in love. Just then he gets all furious and vows he will get an answer from Ishana as to why she chose him, whose heart was recently mended, in order to shatter his fragile heart into a thousand pieces. Whils Om is furious and hurt, shivRu will support him in his decision and make sure Ishana stays the hell out of their brothers life. Shivaay might even insult her by throwing money on her face and Rudra will give her a nice scoldinf too!
