Actually is not coz of Sharanya's pain coz that girl has been crying alot Coz of him but he never felt sympathetic towards her infact he smirks when she cries.. It's her love and concern she towards vyom which broken Shiv's heart and filled it with jealousy to such an extent that he just had to leave that place ASAP!!
See this.. He wants revenge only and only from Sharanya..he would have killed her as soon as she came out of coma but vyom interrupted and he realized sharanya lost her memory..he wants her to remember what she did then he will kill her.

The revenge theory doesn't explain his handhold throughout her coma though! It kinda makes Shiv as a ghost with a bipolar disorder!! That's too much for one actor...gosh!! get him more pink bills producer!!!

I think she left him for vyom..hota haina friend turns lover movies mein..pehle kisi aur ko date karte hai but then they realize that it's wasn't just friendship..he might be a good person before but heart break handling and watching your ex gf with someone else might have broken his soul and made him do things.. He hates her but he still wants her. See he didn't even followed VAnya on the terrace..he knew what VAnya did there..so too much for him to handle!
