The story of Rahul and Muskaan starts off with their entry in DMG. Childhood enemies, these two were always at loggerheads with one another, constantly fighting & amusing/frightening the people around them. But slowly they develop an unique relationship, one that at first cannot be given a name. .

Rahul (or kaankajhoora as Muskaan used to lovingly call him), & Muskaan came from the same city. Their families whom knew each other quite well, decided to get them married. Both of them couldn't stand the thought, needless to say and the marriage was off.
Both of them meet again at the Sanjeevani Hospital branch in Mumbai, where they applied for their internships. When Rahul and Muskaan found out they would be co-interns, one could certainly say ''all hell broke loose''. Enemies from the start, these two never left a chance for fighting, & even brawled with one another while on duty. Their quarrels didn't always include the 'nicest' words, & even lead to slightly violent acts. Though they weren't the best of friends at first, it was evident that hatred was not something they had for each other. When Muskaan's parents insisted upon her marriage to a random man, at a time when she didn't even want to think about marriage; Rahul stepped in and saved her by ''marrying'' her. Rahul and Muskaan's relationship started to grow after their fake marriage, because of all the time they spent together. RM started to develop feelings for each other and their fake marriage of a few months had brought them close. Though they continued fighting, the change was clear and the love was apparent. Just as their relationship was blossoming, and a ''confession'' happening, Rahul gets informed of his cancer stricken father and has to make a difficult decision. He confesses his love to Muskaan, who then confesses as well; but when alone, & Rahul leaves to the US to his father.Muskaan, who by then is very shattered and depressed ultimately decides to focus on her career and opts to go for specialization. Rahul returns after months at the Valentines Day Party, surprising everyone. Reluctantly, Muskaan talks to Rahul and they have a romantic dance. Emotions arise, & the two talk at Muskaan's house, ultimately consumating their relationship. This seemed to only complicate things for them more, and their relationship started to experience some pretty hard times when the issue of marriage came and Rahul refused.

Later Muskaan came to know about Rahul's incapability of becoming a father, which was his reason for refusing to marry. Muskaan and the whole Intern gang had been trying convince Rahul that his impotency does not bother Muskaan and they should see each other again. At their break-up party, Muskaan tells Rahul not to leave her and as she told him before she loved kids, but not as much as she loved him. But Rahul, stubborn that he is, made it clear that he wanted to leave Sanjeevni and Muskaan's life because he knew how much Muskaan loved kids and couldn't bare to deprive her of the happiness he thought she deserved. Rahul leaves Muskaan after hugging and kissing her for the last time, and gives her a doll and tells her to give it to her future child.
And thus is the story of Rahul & Muskaan aka RM.
