It caused by the expansion and contraction of the blood vessels in the nose. These blood vessels tend to be very close to the surface of the skin and often react when the temperature changes. When you come into contact with cold air the blood vessels contract, then when you warm back up they expand larger than they were before for a time, and this leads to the red hue on your nose.
