One thought came in my mind -

When Manjari's health was not good, Abhi was all ready to do anything for his mother..Then he can no understand now Akshu's guilt??? Bade papa was almost on death bed...She is shaken.

Akshu loves Abhi..but she yet to realize that she cannot love without him. He has become her world now.

Abhi from the beginning is head over heels for Akshu because he was searching for that pure unconditional love that he never got from his family except form his mother.

While Akshara did get love from bade papa, bade mummy, Mimi..

If Abhi fall for her selfless love for everyone then he should know what is selfless unconditional love...that love has no limit for person, situation, time...That kind of love cannot be bind to one person..Love can only be increased and expanded not limited to someone...

Giving her some time will bring Akshu's realization beautiful...let's see

I don’t think abhi wants to have #1 position Ak priority list but he definitely wants to be equal to Goenkas. When he was falling in love with her, his question was, “Kya tum mujse aisa pyaar kar pavogi kaise apni family apni behen se karti ho?” Not “kya tum mujse apni family/behen se jyada pyaar kar sakti ho?”. I think he already understands what Manish ji is for her. Remember, Manjari had also told him, “ agar mein tuj se naraz ho jaavu toh tuje kaise lagega. Ak hi haalat wahi hai” and he had said, “janta hu”. He even was telling ak, that “hame pyaar aur parivar dono chahiye na?” - he wants her to want both equally..

His insecurity is that if a time comes where she has to pick only one from family and him then she will leave him. And it didn’t help that Ak sided with kairav when he was choosing family and sacrificing his love. Even at that time, he was triggered and went off questioning this “kabhi haan kabhi naa” attitude of Goenkas siblings.

Also, during the conversation, even though Ak was just showing her guilt but it did look like she was regretting her decisions and blaming her love to be responsible for this. “Kisi ki pyaar ki keemat kisi ki jindagi kaise ho sakti hain. Pyaar ki itni badi keemat chuka ni padegi, pata nahi tha.” “Aisa pyaar nahi maanga tha.” These lines are actually very painful to hear from your love. And it does look like regret (although we know that’s not the case, but try to think from abhi POV given her history of choosing family over him)

Had she added, “ mujhe samaj nahi aa raha ke mein family so kaise samjau ke jaise mein unke bina nahi reh sakti vaise mein apne pyaar ke bina bhi nahi ji sakti” (like she was telling kairav the other day while venting out her anger by boxing) then he would have understood that she is being torn and not siding with family. he understands that pressure too because he was put in that position as well (as you pointed out). At that time, he had taken decision for manjari’s health and in his words “zindagi ulajti gai” and he regrets that decision. So he would have helped Ak to deal with that pressure as well. But here he is thinking she will leave him if family will not budge.

Abhi did realize her unconditional love towards her sister..no matter how she is, she loves and cares for Aarohi. She doesn't demand anything for her love. She just gives.
