Going by the promo, we were to believe that Crowley said, 'so, you haven't changed' Dean because of the brilliant way its been edited. BUT if you watch the trailer atleast twice, its not so very hard to catch that it couldn't have been a possibility. Dean going rogue, carrying out a mini massacre and reuniting with Crowley in 40 mins at this particular point of the story. Nope, can't happen or rather the writers won't do it. And hence guess #1.

As for the laugh, add visual and sound effects, place it correctly between the scenes and any genuine laughter is bound to look freakish. And hence guess #2.
