I think that the episode clearly mentioned that Jay was talking to the doctor...he says yes doctor ( in hindi) at the start of the phone converstation.
The twist could be that this could be the doctor who tells about Pia's child's paternity..after all we are expecting it to be about Bani's condition, and when has Ekta done the expected?\\\ 😆
