i think because he believes he loves her and hence would have stayed purely for her sake and that same was not the case with her she was staying cuz she might be feeling guilty or cuz of another reason but love
but he doesn't knows she too has fallen for him i don't think it is only guilt or mere obligation, she is in love with him. it was clearly visible on her face her expressions said it all and when the doc said it was her love that had saved him the smile that spread across her face in a way endorsed what the doctor said.
