Im sure she has to be an integral part of the show, but they will take a break with her for a while. Then lets say she gets injured or something after they break up, and he is the senior doctor so he had to operate on her, and he couldnt save her, so now he feels like maybe he didnt try hard enough to save her because he hated her or whatever.
