I am getting a feeling that Mandira is ill or something and so Karan is in touch with her. He doesn't want anyone to know about this because obviously no one loves Mandira (not even Karan but that's his responsibility). Since he spends much time there Nandani is getting a feeling that he is having an affair

that could be another reason why he can't leave the US.

Yes I also doubt that karan has extramarital affair.

Does mandira exists still? In this season karan not mentioned mandira 's name for even a second?

Could be but mandira & noina both ill at the sane time is a bit unlikely?

Yep it’s a possibility.

It would make sense if he’s unwilling to leave US if his mom is there and well, she’s dying or something and he needs to be there for her. Maybe that’s what’s holding him back. Perhaps Mandira is terminally ill or bedridden or something. Would be interesting to see if they do go there. In the 1st season we know Karan really loved his mom and her truth really devastated him. It was finally his threat of leaving SN forever and disappearing from India to where no one would be able to find him once Payal exposes her having become Priyanka via the plastic surgery that also made her surrender to police for unwittingly killing Teesha in that car accident iirc.

They completely have avoided the Mandira kidnapped Parth storyline here so maybe if they do take it up from her surrendering, this is a very possible tangent. Karan is unable to tell anyone about the truth and he’s probably spending a lot of money and time on her which makes Nandini think it’s an affair but it could simply be medical expenses. Maybe she got out of jail and called him, and he’s finally caved in to talk to her after her trying relentlessly only for him to discover she’s dying or suffering from some grave disease.

Sumeet gave us a hint today that on his birthday aka Wednesday the teased scene between him and Tulsi will air, and he spoke about it being a complete mother son scene where the son emotionally blackmails his mom to come back to SN.

Karan Mandira could be a parallel wherein the script is flipped and the mom is dying but she’s the one doing the emotional blackmailing. Karan may not have the same level of love, respect and even sympathy for her but he’s a good human being and ultimately he would feel indebted to care for her regardless of her behaviour and actions as he would feel it’s his duty as a son even if she didn’t exactly turn out to be a good human being or a mother.

But it could even be Tanya who probably is sick or something and he feels obligated to look after her as the mother of his son. Let’s see which way they go because it could be either of the two.
