I don’t think he does but he probably associates Anupam with Mandira even now subconsciously… I mean hard not too… and there’s that history with her associated with multiple betrayals and making a fool out of Mihir for decades on end.

Given how it has been shown that the kids don’t know anything about Mandira and Gayatri got a thrill out of informing Pari that Karan is her bua Mandira and Mihir’s illegitimate child, safe to presume they know nothing about that sordid history barring the exception of Pari and even she doesn’t know the details only the most important part.

So Mihir in his mind was probably so angry that he was blaming the same blood that runs in Angad’s veins as it did with Mandira. If he had mentioned Mandira’s name here then the attention would have been taken away from the present circumstances so they made him refer to Anupam instead.

Mihir probably has a paralytic stroke or some kind of issues that require him to take regular sessions with a physiotherapist. He doesn’t know who is treating him when someone is finally assigned by his doctor and that ends up being Vrinda.

Slowly he ends up getting friendly with her and even treats her like he would a daughter without even knowing she’s Angad’s wife he kicked out without even looking at her face.

The fact that he hadn’t looked at her face and the way that scene played out tells me it wasn’t random and has to lead to something in the future. Once Mihir knows her real identity he will be forced to confront his own preconceived notions about a girl he never met versus the person he has come to know and grown to adore being one and the same.

Given that there might be a separation between Mihir and Tulsi too post leap it might be possible that the fact that even Noyna hasn’t seen the face of the girl who got Mittali ditched comes into play. She has no clue she has been meeting the same girl and getting Mihir treated by her if Mihir ends up leaving SN owing to his growing differences with Tulsi and the possible/supposed ONS with Noyna that’s coming up.

People who work out regularly may require a physiotherapist for faster recovery from muscle cramps and injuries. Since Mihir is known for his physical fitness, we can expect him to suffer injury and consulting Vrinda probably. The leap might have diluted the wounds too. So with the new impression, Mihir can hopefully develop a father-daughter bond with Vrinda.

If they are really taking leap, probably Paridhi will be suffering under Rannvijay's clutches during the leap duration. It will be sad as we cannot see Paridhi's suffering daily, for whatever she had done to Ajay.

I was thinking if Mihir will leave Shanti Niketan or Tulsi will leave Shanti Niketan. Because Mihir once admitted that SN cannot run without Tulsi so he left the house. If Mihir leaves SN, Vrinda and Angad can stay in SN.

Bold - Noina has seen Vrinda's face in the karwachauth video that Mitali shown to her. Seeing her face, Noina told Mitali that she looks unwell. More importantly, she saw her in the video where Tulsi was slapping Angad.

I’m speculating here but we have had a history of this happening.

Shagun moved in with Raman and his family post a leap in YHM under the excuse of looking after the children. Here Mihir has proclaimed himself as the man who is with the family of these 3 women and for all we know the writers make him dumb enough to move in with them to protect them from some taunts that they can avoid by simply shifting back to the US but will stick around to hear just so that Mihir can destroy his marriage and family and Noyna gets her fantasy.
