Mannat would have left if Sunita dies or she thinks she is a threat to Vikrant... but with this health scare would she go?. Suwati said this in IV that Mannat will get to know someone is drugging Vik but not who... that should be enough impetus for her to stay back.
Apart from Harneet... in the ice balm scene... Mannat looked at Vikrant when she said "koi mujhe ya mere Apne logo ko kyun chot pahuchana chah raha hai"... she looks at Vik when she said apne... definitely considers him as one of 'her people'.... Mannat won't be leaving Vikrant in trouble even if she suffers!!!! There are no 2 ways about it.
