If you find a cockroach IN your food, do not walk, run away from that restaurant. Not only are there likely hundreds of cockroaches in that kitchen, but the kitchen staff and management are ambivalent about the problem. Cockroaches are nocturnal insects. They prefer to live and feed in the dark, so a cockroach seen during the day usually the sign of infestation.
If you are a manager in a restaurant and you are presented with this problem, you should apologize profusely, make sure the guest doesn’t pay for a single thing and ask if you can get their phone number to follow up with them. Make sure they know that you are taking the problem seriously and will have pest control out as soon as possible.
Any restaurant I’ve ever run has had a bulletproof pest protection plan with a reputable company. Cockroaches usually never made it in the restaurant, but if they did our pest control company would be out the same day and repeatedly after that to ensure the problem was solved. A reputable restaurant owner will have a monthly inspection and prevention plan set up with a pest control company,
The only exception to this is patios. I once ran a restaurant with a large and popular patio. Even though our pest company would regularly spray to prevent cockroaches from climbing into the patio, occasionally one would make it’s way up there.

That depends on the customer. Myself I'd be right to management. Then more than likely to the Board of Health. You don't just see one cockroache anywhere from what I understand and have seen. They are hard to control and kill. They multiply like there's no tomorrow. They can live in a microwave that is turned on. From my experience, they stop eating, refuse to pay for the food, get up and leave, never eat there again.
While this has never happened to me personally, I have twice been dining with friends when someone else found a dead cockroach in amongst their food.

I can tell you in an Ethiopian restaurant here in D.C., if you find one in your espresso, they take a fork, dig it out and hand you back your cup.
If it happens elsewhere, just aside from dying of grossness and embarrassment, that able eats for free with apologies up and down the chain of command.
That's why there are those people, who actually bring bugs to restaurants. I’ve seen it 2ce. I’ve also seen someone pull a blonde hair out of a baggie. We had no blonde employees in the entire house. But.. God forbid we call a guest on something like that.

How would you handle a customer who is complaining about finding a cockroach in his food served to him at your restaurant?
I myself would handle the situation very delicately. First, I would apologize to the customer. I would also be taking notice of how much of the food had been eaten before they found the roach. If the entire was devoured before they reported it, I would have questions. If they were just starting their meal, I would be so apologetic and would offer them a free meal at another time and not charge them for the current one.

I would signal for the waiter and quietly point out the fact that there is a cockroach in my dish. Ideally he would call over the manager and he would apologize and offer to replace the dish. I would decline the offer and ask to have my bill zeroed out (except perhaps any drinks I had)., then leave. I would also use my cell phone to snap a photo of the roach in my food. If the manager gives me a hard time about giving me my money back, that experience and photo would be used on Yelp, Trip Advisor, etc.
