I don't know if this is the case in India, and if it is it's a sad state of affairs.
When BP is elevated emergency C-sections are safest options. I had 2 friends who had that, everything was normal, she went for her routine checkup and doctor said your BP is elevated and immediately did C-section within the hour literally, I mean she took just an hour off from work to stop by the doctor and little did she know she'd have a baby in 2 hrs!
So yes things happen is totally normal cases too.
I don't know about charges and all, but to me health/safety of mother/baby is most imp. Really cannot comment on doctors promoting C-section over normal.
I do know some friends who knew they were having C-sections due to whatever reason, placement of baby or age or whatever, they discussed, planned it we'll ahead in time.

Again just relaying what is in US from experience, I don't know the norm in India.
