So Jaan-E-Alam is this young, good-looking, successful businessman who loves his mom and sister. He seems to be the ideal son and the perfect respectable man, who even looks after the help in his house as his own family members...no eve-teasing or harassment allowed. But in reality he is a playboy and a womaniser who can exploit anyone for his needs.
He has a secret girlfriend, Noor-u-Saba in Karachi (his family doesn't live there, nor do they know about her) and she calls him frantically to tell him she is pregnant. If there's one thing Alam loves more than his family is having a respectable position in society and having a child out of wedlock is well...shameful. Plus he doesn't love her, she's just a f**ktoy for him. Noor-u-Saba of course is head over heels, since she doesn't know his evil side. He tries to get her to abort the baby but she blackmails him to either marry her or she tells the world about the baby. He goes to his doctor friend, Hadeed to arrange for an abortion. Hadeed is upset because he's done this for Jaan-E-Alam multiple times in the past and wants him to marry Noor-u-Saba. Alam reveals that if Saba slept with him, she could do it with anyone else and that's characterless..."usse mann bhar gaya hai", whereas the fact was that he seduced her in order to make their sexual relationship consensual.
At the same time, Alam bumps into Mariyam (Sanam Chaudhry), who belongs to a very poor family. Her father is suffering from a heart condition and she needs money for his surgery. Her mother teaches Qu'ran to children, if I remember correctly. Now Alam spots this innocent Mariyam working at a perfume shop and makes her his next target. He creates such circumstances along with his driver, Malik that now Mariyam starts working at Alam's office.
Alam also manages to trick Noor-u-Saba into thinking his mother's coming to meet her for marriage talks, but convinces her to get an abortion because a child before marriage wouldn't be acceptable to Alam's mom. Noor-u-Saba goes through with the operation...then finds out Alam lied to her. He fires her, kicks her out of the apartment she lives in and gives her job to Mariyam. Saba manages to warn Mariyam about Alam's behaviour but he manages to manipulate the situation. Saba commits suicide and Alam pays off the police inspector to close the file.
Mariyam now gets a loan from Alam for her father's surgery but its a scam since she signed a blank affidavit and he changed the money amount owed by her from 5 lakh to 50. She doesn't know this yet though. A corrupt Minister has some business deals with Alam and in order to make that pass quickly, Alam offers Mariyam as a "gift" for one night at Alam's farmhouse. Mariyam manages to run away but is now being blackmailed by Alam to either sleep with the minister or arrange the money. Her father passes away from the trauma that his daughter did not return home all night.
Mallik has a change of heart and he ends up helping Mariyam and her mother. The two reach Alam's mother's house and reveal his truth to his mother and sister on his sister's engagement day. The engagement doesn't go through and Alam faces humiliation.
Jaan-E-Alam, having lost the contract and having paid so much money because of Mariyam, now vows revenge. He does an entire setup with sending paid actors to the new locality where Mariyam, her mother and Mallik have moved to. One of the actors is "betrothed" to Mariyam. But on the wedding day, he gets these pictures of Mariyam sitting with the Minister and breaks off the marriage. Alam turns out to be the knight in shining armour, offering to marry Mariyam. He brings his now-wife to his farmhouse, where she gets traumatised by the memories, but they end up consummating their marriage since he's being so sweet and seemingly changed.
On the day of their Walima, he promises her he will never hurt her again so Mariyam decides to forgive him. Alam then humiliates her character in front of everyone, making suggestive statements like he knew on the wedding night itself that she was not a virgin, thus maligning her character and divorcing her.
A shocked Mariyam then screams as a gloating Alam is leaving the venue "Jaan-E-Alam! Ab Tu Dekh Mera Khuda Kya Karta Hai!"
Mariyam's mother passes away and then there's a time jump, the second half of the story focussed on the effect of Mariyam's curse.
