May be that's why they had to fire her cos it was a avoidable mistake and a very novice one.
It's rare but not unreal.
It famously happened with a Pixar movie. The only 'backup' they had was an accidental copy on an employee's personal drive. It happened with a friend of mine too, but luckily a week before the deadline.
If it can happen to billion-dollar giants, it can surely happen at mid-scale businesses too.

Yes, you are exactly right about getting prompt before the actual deletion of the file.
That's not default in the latest Windows & Apple OS. You have to change settings to activate delete confirmation prompt.
