How did Shiva get that bruise on his neck?

How did Shiva get that bruise on his neck? Was it during the beating?

I really hope that wasn't from getting hit, especially since the location of that bruise is near his airways/throat, both of which are not protected by bone. Based on the way Dhara was hitting, it would've seriously injured Kanwar if she used that much force at his neck.

I'm thinking Kanwar had a fungal infection/insect bite (especially since they seemed to have gone hiking while in Bikaneer), and he is just recovering from that.
