Would be good if it is nok jhok because Sai is yet to know that the equation between the two has actually changed. She will be talking like how she always used to be before she left the house. If Virat doesn't like something, he should be frank about it and I am sure Sai will understand it and won't do it if he is serious about it.
Pakhi won't stop from entering Sairat's bedroom and if Virat continues to be act the same way as before, not raising objections or not being stern with her all the times, Sai won't be able to understand anything since everything has forever been like this since the start.
Sai brings her up as a joke and also to remind herself at times that there is someone named Pakhi in their life. Pakhi is a jeethi jaathi reminder who challenges Sai as a wife and Sai fails to understand a few times when Virat stays silent. So the more he stays silent when Pakhi does something, the more there will be misunderstanding between Sai and Virat.
I feel Sai sometimes jokes to even know what he thinks about Pakhi and if he is really serious that he doesn't like her. Like how Virat had brought the nurse yesterday to make Sai come home, Sai might use Pakhi's name to assess his feelings or his reaction.
Even in last night's episode I didn't think Sai bringing it up was bad because it was the same like how Virat asked if you don't like any woman touching my body.

To Sai, Pakhi or nurse or any other girl is almost going to be the same. Pakhi is a person she knows from her everyday life and who also always wants to stay near Virat.
Yes, Sai does know they have a past, but she somewhere feels that it is not their present. She is just not sure about it.
People do tease their spouses with their favourite actresses or their colleagues or someone whom they have shared as having crush with and it is all harmless. And if one doesn't like it, they will be frank about it and that is what Virat has to do if it is really bothering him a lot.

I appreciated that Sai thought (in front of the mirror) as what went wrong and quickly understood that he probably didn't like her bringing up Pakhi's name. Ashwini also told her the same the next morning and she corrected her mistake by taking food for him on her own the night he was at the hospital. She didn't even let Pakhi manipulate her at the terrace when she was trying to call Virat and when he didn't pick her call.
But when Virat brought up Pakhi like that for food or goes out of the room with Pakhi or let Pakhi talk about their trip as if it was their honeymoon, not letting Pakhi know that Sai is going to join them and so much more, it is going to be difficult to remove Pakhi from Sai's head without him clarifying about his relationship with Pakhi to Sai.
When Virat said I don't know what Pakhi wanted after Pakhi left the hospital room, Sai answered, is it not obvious that she wanted to stay with you and take care of you. Sai had even asked him to let it go in an attempt to change his mood since he was upset.
Sai asked Virat to let it go because she understood the gravity of the situation that Pakhi kept insisting on staying with him even after he asked her to leave which he didn't like.
She heard what Pakhi said before leaving the room which upset Virat. At that point of time, she acted matured and asked him to let it go and not get worked up since the situation was serious.
But she brought it as a joke later thinking it wouldn't be anything serious. They were in their regular funny mode, so she didn't think that could be harmful.
Many in the forum joke about Pakhi so that their BP don't rise, I think Sai is also probably doing the same.
I think it will be funny if Sai brings her up, she probably can and should look at Virat's reactions and know when to stop like how she did yesterday. It was very cute and it looked like it was also helping Sai develop an understanding towards how Virat feels. They both can joke together too many times to come out of a serious situation.
