not only is Abir physically drained after his horrific accident, but he is now emotionally reaching his limit. He's had enough with trying to indirectly and often quite frankly expressing his sincere feelings infront of Mishti but more importantly trying to make Mishti realise her own emotions.
By this point in their story; he himself has finally come terms and completely accepted his feelings for this girl after much difficulty. And carefully explored the possibility of her reciprocating similar feelings back. With the responses and reactions he received - he accordingly has made countless attempts to potentially get Mishti herself to break that wary barrier between them and admit her feelings openly to him.

That's why after such a grave yet thrilling incident; Abir realises that Mishti's valiant act last night is now his biggest strength - he thinks that's it, I'm going for it right her right now, straight to her face, it's time to reveal the secret of the painting-wali
That determination and sudden build up of courage is evident through those crystal shining brown eyes. But the shaky breath and nervous swallow, still conveys his underlying anxiety/apprehension - he knows exactly how risky this is. By demanding Mishti for answers she herself is yet to comprehend, he knows exactly how unstable this tight rope he walks/balances on is. But he takes that risk, he walks across that black line eyes closed; by holding her palm tightly in his, looking straight into her eyes and firing those questions at her bolo mishti! mere piche kyu aayi thi?
His heart beating out of his chest and ears intently alert in hoping to hear that same answer he felt when Mishti saved him the first time (saving his painting in the fire) = "ISHQ"
