I don't think it is runaway groom but Om coming to tell Shivvay some bad news. Maybe Annika called OM because Shivvay's phone is obviously unreachable since he is getting married. In her hurry to reach there she may meet with an accident or something and Om might only hear her scream before the phone goes dead.
She may have told him where she is off to and asked them to meet her there before the accident happens
Om's look is one of fright upon getting bad news. He knows Shivvay cannot bear Annika even hurting her knee - so how then is he going to live down Annika dying on his wedding day only to make things right in his life.
Of course she does not die - we all know that but maybe an accident severe enough for Shivvay to go berserk and breakdown.

I feel Anika will make a grand entry like Mallika 😆 But maybe got into a little accident while she was on the phone with Omkara I guess that's why he look serious in the precap.
