Shivaay has been parent to rudra and how much he loves him it's always evident in the things he do it for shivaay.

Actually on rewatching the scene even I felt it was not a comparison but just a general statement to show his love. Had Om been hospitalized, he would have said something similar for him as well

On repeat watch I too feel that we are reading too much into his statement . It's not to be taken literally. All that he was trying to say was that though Shivaay is his first cousin the relationship is so much more beyond that.
