For now, Shagun has been Pihu's mom, so it would be traumatic for a child to suddenly not call her mom "mom"

I'd like to think in the future, once Shagun crosses the line too far with Pihu, that Pihu one day calls Ishita "maa". Afterall, Adi and Ruhi never call Shagun "aunty" because she is their biological mother at the end of the day. It should be reasonable and not extraordinary for Pihu to be able to do that with respect to Ishita.

True she will definitely leave after ruhi's anger subsides n only person who can stop her then would be raman guess till then we will just get lollipops in between a lot of drama n torture since now nidhi as well as shagun both are there might be join hands also as in today's episode again shagun was not at all looking happy when pihu n everybody else were praying for ruhi to come back as she knows since ruhi's truth is out there is a change in ishra's equation n now that ishita is coming back to bhalla house so situation will change completely.
